Why do we have to sign a consent
form?
Consent forms are required by the Institutional Review Board
to inform the participants of the study about the procedure
approved by the Board for handling the data (especially protect
confidentiality), and about the participants' rights. Participants
who agree to have their data analyzed should read and sign
the consent forms.
Here is the consent form
that you will be asked to sign for the Leadership Game. The consent form for the survey will be accessible at the beginning of the survey.
Is participation in the survey
and in the study mandatory?
Participation in the survey and in the Leadership Game is
part of the self development initiative and all 1Y MBA students
are asked to participate. However, participation in the study
is not mandatory. For each portion of the study, you will
be asked for your consent to release the data for research
and analysis (in order to receive your personalized feedback,
we need to analyze the data). You may decide whether to consent
or not.
If
I decide not to consent, can I still receive my personalized
feedback?
No. If you
decide not to consent, we can not, by law, analyze your data.
Thus, we cannot give you any personalized feedback.
If
I decide not to consent, do I still get paid depending on
my performance in the games?
Yes. You
will be paid at the end of the LEADERSHIP GAME SESSION, depending
on your decisions and the decisions of other participants
in the game, whether or not you consent.
If I consent to release data
for one part of the study (e.g. the survey), do I automatically
consent to release the other data collected in this study?
No. There are separate consent forms for each part of the
study (survey and games). The consent form for the survey
will appear in the first page of the survey. Separate consent
forms will be handed to you during the LEADERSHIP GAME SESSION.
If I do not consent to release
data for the survey, and then I change my mind, because I
realize that I want my individualized feedback from you, can
I reverse my decision?
No. Unfortunately, you can not reverse this decision. If
you decide not to consent, we can not provide you with individualized
information and you can not reverse your decision.
If I consent and then change
my mind, can I withdraw from the study?
Yes. You are free to withdraw at any time. At any time in
the future, including after your graduation from Kellogg,
you can withdraw from the study or portions of it by contacting
