Report
of Candidacy
All
part-time students who plan to complete their Master
of Business Administration degree requirements in the
2007-2008 school year (September 2007 - August 2008)
should file a Report of Candidacy.
Please
note that the PDF document is not an electronic filing
system. Once you have completed the form,
you either can drop it off at the Part-Time MBA Program
office (340 East Superior Street, 2nd Floor Mezzanine,
Chicago) or fax it to 312-503-6530.
- The
Report of Candidacy Form is two pages and is designed
by using text boxes, so you can type your information
directly onto the form.
- Select
the document via the link listed above. You will be
unable to save a completed copy to your computer
unless you have the full version of Adobe Acrobat--
therefore, complete the form and print it. You
will need an active NetID and password to access the
form.
- To
complete an area, simply place your computer mouse
and click the area next to the question or item.
A text box, indicated by a flashing cursor, will allow
you to enter the necessary information.
- In
areas where there is a check mark for an answer, you
simply need to click for a check mark to appear.
- Please
complete both pages.
- Remember
to sign and date the form before turning it in to
the Part-Time MBA Program office. If you have

A
quick distinction regarding "June Grads"
and "August Grads"...
The Part-Time MBA Program separates their graduating
students into two groups, June and August Grads. Students
who will complete their MBA degree requirements in
the Fall, Winter and Spring Quarters are considered
"June Grads". Students who will complete
their MBA degree requirements in the Summer Quarter
(after the Convocation Ceremony) are considered "August
Grads."
Both June Grads and August Grads are able to participate
in the Convocation Ceremony. Together, you are all
members of the graduating Class of 2008.
