Application Procedures

* NOTE: MARCH 31, 2005 WAS THE PREFERRED DEADLINE FOR OUR OFFICE TO RECEIVE YOUR FINANCIAL AID MATERIALS. HOWEVER, WE CERTAINLY WILL ACCEPT APPLICATIONS AFTER THIS DATE. ALSO, ONCE WE RECEIVE YOUR COMPLETED APPLICATION MATERIALS, YOU SHOULD RECEIVE A PACKAGE WITHIN TWO WEEKS.

The Kellogg Office of Financial Aid will begin making financial aid decisions in late March. Our office will make awards continuously for admitted candidates whose required forms have been received. For full consideration in 2004-2005 with regard to Kellogg grants, all of the following documents must be submitted by May 1, 2004.

Two-Year MBA and MMM

1. U.S. Citizens and Permanent Residents only need to:

Complete the Free Application for Federal Student Aid (FAFSA)

All U.S. Citizens and Permanent Residents are required to complete the "Free Application for Federal Student Aid" (FAFSA) for the 2004-2005 school year. Please remember to enter the following information in "STEP SIX" (Questions 87-97) of the FAFSA.

U.S. Citizens and Permanent Residents may submit the FAFSA by using ONE of the methods listed below:

We recommend that you file the FAFSA online here. If you use this method, there are two methods of providing a signature. You may 1) contact FAFSA and request a PIN number (your PIN number is your electronic signature) or, 2) you must print, sign, and mail to the FAFSA processor the certification page which is provided once you have completed your online application.

Before closing your application, please print a copy and include it with your Financial Aid Application. Please note: once you close your FAFSA online application, you will not be able to return to it and print.

If you cannot apply online, you may obtain a paper FAFSA by calling the Federal Student Aid Information Center at 1-800-433- 3243. If you use the paper application, you must mail to Kellogg a copy of the FAFSA application that you have mailed to the FAFSA processor to be eligible for processing.

2. All students (U.S. Citizens, Permanent Residents and International):

Complete the Kellogg Financial Aid Application
Please complete and submit to the Kellogg Financial Aid Office the "Kellogg Financial Aid Application, 2004-2005" if you want financial assistance.

3. All students (U.S. Citizens, Permanent Residents and International):

Signed 2003 Income Statement

U.S. Citizens and Permanent Residents: submit to the Kellogg Financial Aid Office a SIGNED copy of your (and your spouse, if married) 2003 federal income tax return (1040, 1040A, 1040EZ, 1040TEL) and all schedules (if applicable). If you filed electronically or an outside agency completed your 2003 federal income taxes, please still sign the photocopy of the 1040 form you are submitting to our office. Please DO NOT submit your STATE income tax returns. If a request for an extension to file the 2003 federal income tax return has been filed by you(and/or your spouse, if married), a copy of that request must be submitted along with copies of all W-2 forms. Upon review of other application materials, provisional financial aid decision may or may not be made with this information pending receipt of the official tax return(s).

International Students: submit an income statement and/or letter from your employer(s) stating your 2003 income and taxes paid (and your spouse if married). The dollar amounts listed on this form/letter must be converted into U.S. dollars.
