Anyone who is interested in the Kellogg Executive Scholars Program should complete this form. This form serves to inform the school of your intention to become a Kellogg Executive Scholar. It will help the school monitor your progress and serve as a tool to be used by you and the Executive Scholars Program Advisor in order to ensure that all programs in your track will meet certificate requirements.
The form is non-binding and does not obligate you to attend any programs. Your program choices can change over time, as long as they still fit within the curriculum guidelines.
