Newly enrolled Northwestern students are required to follow the admission health requirements of Northwestern or a hold will be placed on their fall registration. Please refer to the Health Service Department's Web site for details.
- Print and complete the Admission Health Record for Non-Healthcare Students Form. Instructions for completion are listed on form.
- Have your healthcare provider complete "PART II: Required Immunizations" in the Admission Health Record or submit copies of previous immunization records per the instructions.
- Review the required immunizations for non-healthcare students, which is also listed on the Admission Health Record.

Students should

it by July 1, 2013. Penalties for
not fulfilling health requirements include a fall registration hold
and a non-refundable $100 late fee.
